Nathan offers advice and education to empower his patients in the management of their condition. Exercise therapy, the foundation of our treatment approach to restore normal function, this can be either home, pool or gym based. Manual therapy including joint manipulations and mobilisations to get things moving, when they are stuck. If you do not, we can refer you to see one on a self-pay basis. Ideally you will be signed up as a member on an annual monthly subscription plan. However, you can register with us as a Pay As You Go patient.

No, we can only issue you with a private prescription that your pharmacy will process in the same way. Some medications are cheaper on a private prescription than with the NHS and others are more expensive. This can vary from pharmacy to pharmacy. There are over 100 varieties of Human Papillomavirus (HPV) and 4 out of 5 people will come into contact with at least one type of the HPV virus at some point in their lives. Most forms of the HPV infection are harmless, however some can cause genital warts and abnormal cells that are linked to cervical cancer and other cancers in men and women. What is the HPV vaccine? The HPV vaccination (Gardasil 9) protects against 9 types of HPV, including 2 types that can cause more than 70% of cervical cancers in the UK and 2 types of the virus that cause 90% of genital warts. Gardasil 9 is given as an injection in the upper arm. The number of doses you need depends on your age. This is a booster vaccine which is used to give children immunity to diphtheria, tetanus, pertussis and polio. The aorta is the main blood vessel that supplies blood to your body. It runs from your heart down through your chest and abdomen. In some people - as they get older - the wall of the aorta in the abdomen can become weak. It can then start to expand and form an abdominal aortic aneurysm. The condition is most common in men aged 65 and over and screening on the NHS is not offered to anyone not in this category (i.e. men under 65 or women of any age). If you have an aneurysm, you will not usually notice any symptoms. This means you cannot tell if you have one, will not feel any pain and will probably not notice anything different. Fylde Private GP reserves the right to refuse to provide advice or consultations where: Membership fees have not been paid Outstanding charges have not been paid In its reasonable opinion, the giving of service would involve any breach of the law It reasonably considers that you or anyone accompanying you, is behaving or has behaved in an abusive or threatening manner to Fylde Private GP’s employees or agents It reasonably considers that that you, or anyone acting on your behalf, is behaving, or has behaved, in an abusive or threatening manner to Fylde Private GP’s employees, either over the phone or in writing Please note: if you are refused service by Fylde Private GP you have the right to an explanation in writing (see ‘Compliments and complaints’ for contact details).

Fylde Private GP is entitled to change any of the Terms and Conditions on the anniversary of your joining date. Fylde Private GP also reserves the right to make changes to these Terms and Conditions during the Membership Year, on the giving of reasonable notice, where it reasonably considers this necessary in order to comply with any applicable laws, regulations or the instruction of a regulatory authority.

You can cancel or reschedule appointments up to 24 hours before the appointment. Fylde Private GP shall not, in any event, and to the extent permitted by law, have any responsibility for any increased costs or expenses, for any loss of profit, business, contracts, revenue or anticipated savings or for any special, indirect or consequential losses incurred as a result of or in connection with any service, whether resulting from tort (including negligence or breach of statutory duty), breach of agreement or otherwise. For the avoidance of doubt, nothing in this clause or these Terms and Conditions shall exclude or restrict Fylde Private GP’s liability for negligence resulting in death or personal injury.

Changes to your name, address, telephone number or bank account details must be notified to Fylde Private GP immediately. You can log into the Patient Portal and make these changes yourself or alternatively you can call us during practice opening hours and we can make these changes on your behalf.

You have the right to cancel your Membership Plan within a 14 day ‘cooling off period’, commencing from the date of your membership and initial payment. Cancellations must be confirmed in writing.

i. These Terms and Conditions are valid for patients using our service on a Pay-As-You-Go basis on or after 01/10/2021. Please read this information carefully and keep it in a safe place as use of our services are subject to these Terms and Conditions. ii. Pricing Please see our website for our Pay As You Go Pricing. Appointment Cancellation Policy You can cancel or reschedule appointments up to 24 hours before the appointment. Members are encouraged to continue their registration with an NHS GP if they are entitled to NHS care. The clinic will liaise with any other healthcare professionals involved in your care, with your consent, as needed.

If you choose a video consultation, please note that you will be required to download the Meddbase Telemedicine Application to your phone, tablet or PC prior to your first consultation. Once you have made a video consultation appointment, you will be sent an email and text confirming the appointment along with a link to join the consultation. Please ensure that you have downloaded the app before the start of your scheduled appointment time so that this does not delay the start and leave you with less time for your consultation. It is the patient’s responsibility to ensure that this application is downloaded in advance and Fylde Private GP cannot be held responsible for missed or delayed consultations. The day before your telemedicine consultation, our staff will call each patient to remind them of this requirement.

Please note that 15-minute appointments are for minor alignments only. We highly recommend contacting the practice before booking to assess your requirements to confirm the most appropriate appointment type and length. Please note that if your appointment is for a complex issue and overruns the allocated 15 minutes, you will be charged an additional £45 for a 30-minute appointment.

ii. If you have already had a consultation, you will receive a full refund of the Membership fee paid. iii. If you during the 14 day cooling-off period, you have already used any of our services, you will receive a full refund of the total Membership fee paid less the cost of any charges for our services based on our Pay-As-You-Go Pricing. iv. If you have during the 14 day cooling-off period you have used our services and the cost of these services used are in excess of the Membership Fee paid you will be responsible for paying the cost of the difference between the two. Your Membership and these Terms and Conditions are governed and should be interpreted by the laws of England and Wales.

Before your first appointment, we will ask you to fill out a health questionnaire. For your safety, this must be filled in as accurately as possible, including details of all your existing medication. To complete this health questionnaire, please log into your member's portal here, go to Questionnaires and complete the questionnaire. Once completed, this will be submitted through the system and will be saved in your patient notes ready for the Doctor to review with you at your first consultation. While Fylde Private GP seeks to meet the service needs of members at all times, its resources are finite and this may not always be possible. Fylde Private GP shall not be liable for service failures where Fylde Private GP is faced with circumstances outside its reasonable control.

Any additional charges for services not included in the consultation will be advised to patients before they are requested by the clinician. An invoice detailing these charges will be raised during the consultation and will be ready for payment before you leave the practice. The prices for individual items are also available through our website or by contacting our reception. If you have any queries or questions regarding fees, please do not hesitate to ask for clarification. Please note additional fees may be incurred for vaccinations, laboratory fees, procedure fees etc. All such fees are payable immediately after the provision of service.

If you do not cancel or do not attend the appointment without informing us, you will lose one of your included annual consultations from your membership plan.

v. The minimum term prior to cancellation for annual membership plans is 6 months. There will be no refund of the Membership fee for the first six months. The only exception will be in the event of death, in which case we may refund your estate with an amount proportionate to any full months left. vi. You can cancel your cover at any time after the initial 6 months by giving a minimum of 30 days notice. Payment cancellation will take effect at the next payment date following the end of the notice period. You will be advised of the date when your membership expires. vii. Fylde Private GP has the right to cancel any membership plan immediately if you or anyone accompanying you, is behaving or has behaved in an abusive or threatening manner to Fylde Private GP employees or agents, or if, in Fylde Private GP’s opinion the therapeutic relationship has irretrievably broken down or it is clear that the service expectations and demands of the patient are unreasonable in relation to the services provided by Fylde Private GP. A CT Cardiac Calcium Score scan is a simple Computerised Tomography (CT) scan to check for deposits or build-up of plaque on the walls of your coronary arteries (arteries that supply blood to your heart). It doesn’t provide detailed pictures of the heart or heart arteries but instead measures the amount of calcified, or hardened, plaques in the heart arteries. Plaque is made up of calcium, fat and cholesterol. This is usually explained as a calcium score which provides an assessment of the number of fatty plaques in the heart arteries. A higher coronary calcium score suggests you have a higher chance of significant narrowing in the coronary arteries and a higher risk of a future heart attack. This test gives your GP early warning of the narrowing of the arteries and the risk of a heart attack.
